Thank you for reaching out to the Transgender Legal Defense & Education Fund (TLDEF)'s Name Change Project.

We assist in connecting LOW-INCOME TGNCNB (transgender, gender non-conforming, and/or non-binary) people with law firms that provide them with pro bono (free) legal representation for name changes in the service areas listed above.

COMPLETING THIS FORM

It may take up to 30 minutes to complete this form, and you will need to reference documents such as your birth certificate and photo ID, if you have them, in order to complete it. The information you provide in this form will be the starting point for your name change petition, if you are eligible to be matched with an attorney through the Name Change Project. Taking the time to provide complete and accurate information in your intake form will help ensure that the rest of the process goes as quickly and smoothly as possible.

WHAT HAPPENS AFTER I SUBMIT THIS FORM

Once you submit this form TLDEF will review it and determine your eligibility for the Name Change Project.

If you appear likely to be eligible for assistance, you may be contacted to schedule an intake call with one of our associates, or you may receive an email update from us that you have been approved and referred directly to a cooperating law firm. If you are ineligible, we will reach out via email to decline your application, and we will provide self-help resources for you whenever possible.

CONFIDENTIALITY

The information you submit via this form will be considered by TLDEF in matching you with a cooperating law firm. It will only be shared with cooperating law firms who we have determined may be able to assist you in changing your name. Except for providing your information to cooperating law firms, TLDEF will keep the information you share with us confidential.

NO LEGAL REPRESENTATION BY TLDEF

Filling out this form does not establish a lawyer-client relationship with TLDEF. TLDEF does not take on name change clients directly but operates the Name Change Project in partnership with private law firms and corporate in-house counsel in the above-listed cities and counties.

If TLDEF determines that you are eligible for assistance, you will be represented by your assigned lawyer for your name change only. If you do not live in one of our service areas, the lawyers we work with will be unable to assist you with your name change, but we may be able to refer you to other resources.

TLDEF can only accept requests for legal representation directly from the person seeking legal assistance. However, if you are the parent, guardian, or "next friend" of a TGNCNB youth or the case worker for a TGNCNB patient, you may fill out this form on their behalf.
